Laser Eye Surgery


Although originally only for short sighted (myopic) patients up to –6.00, laser eye surgery can now be effective for high prescriptions as well as long sight (hyperopia) and astigmatism. The type of laser eye surgery best suited to each patient can vary according to corneal thickness and shape, but in general the most popular laser eye procedure is LASIK (using a corneal flap), followed by PRK or LASEK (surface treatments).

Blade Free Femtosecond-LASIK
Lasik combines the use of two technologies: the femtosecond laser  to create the flap, and the excimer laser to  treat the corneal bed where it re-shapes the cornea and thereby changes where the image focuses on the retina. Once treated by the excimer laser the flap is then laid back down and stays in position until healing is completed.

In the past, the corneal flap was created using a mechanical blade which was done prior to the laser correction itself.  Millions of such corneal flaps have already been created using this technique but often many people decide not to undergo this procedure because they are uncomfortable with the idea of their eyes being touched by a blade.

For several years now it has been possible to create the flap using a femtosecond laser. This means that no blade is involved in the procedure. Patients feel more comfortable, and the result is even better. Over three million patients have already undergone bladeless laser eye surgery.

Z-LASIK: The future of bladeless laser surgery

At Kirkwood Fyfe, we use Z-LASIK which is the most advanced innovation in bladeless surgery. What do ALL LASER Femtosecond-Lasik Procedures have in common?
Femto LASIK has been available for more than 5 years, and more than 3 million femto LASIK procedures have been performed successfully worldwide. The major advantage of femto LASIK over conventional LASIK is that the entire vision correction treatment is performed by a laser. The use of blades is no longer required.

Z-Lasik offers many advantages that lead to better results:

Faster Visual Recovery
Fast result: The recovery period is shorter than for other vision correction techniques. Visual improvement is noticeable within a few hours.

Total procedure time is shorter
The surgeon controls the optics of the laser, increasing accuracy and reducing contact time with the eye.

LDV technology uses an incredibly fast sequence of tiny light pulses to generate the smooth surface in the corneal tissue that is required for optimal vision correction. Within the few seconds required to create the corneal flap, the LDV will place around 400 million tiny, overlapping laser spots inside the cornea.  Z-LASIK has an extremely low impact on the eye: The procedure takes only a few minutes. The time the laser is in contact with the eye is reduced to a minimum.

Thinner Flaps
Thanks to the extremely high frequency of the laser pulses, the layers of the cornea are separated with unprecedented precision, creating an ultra-thin flap. Z-LASIK is therefore effective in patients who were previously not candidates for laser surgery, including those patients with extreme myopia or thin corneas.

Due to the high precision of the Z-LASIK procedure, thinner more predictable flaps can be created. Higher levels of myopia, astigmatism, and hyperopia can be treated with confidence.

Larger Treatment Areas
Z-LASIK enables the surgeon to enlarge the treatment zone which allows for improved quality of vision, especially at night or in poor lighting conditions. Few patients have experienced halo effects with refractive procedures, but with Z-LASIK, this undesirable phenomenon is even rarer.

Reduced Occurrence of Dry Eye Syndrome
Dry Eye Syndrome is a minor side effect of LASIK (it is observed in 4% of all treated eyes). With the gentler Z-LASIK procedure, the incidence of Dry Eye Syndrome is further reduced.


Reduced inflammation:
Z-LASIK technology uses light pulses with much lower pulse energy. Higher pulse energies used in other technologies can cause inflammation which may affect your vision and may cause pain for a few days. Such inflammatory response can be treated with medication and will not cause a permanent harmful effect. However, in Z-LASIK no such adverse reactions have been reported.

Surface Treatments

These do not involve the creation of a flap, but only the removal of the surface layer of cells (epithelium). A new layer of surface cells takes 4-7 days to re-grow, so the recovery time is longer. However a surface treatment may be indicated if there is a very thin or irregular cornea.

Laser Eye Surgery - Your questions answered

  Is Laser Eye Surgery safe?    
  Several controlled clinical trials have been conducted to evaluate the safety of Lasik. Following these studies, laser eye surgery systems were approved by the FDA as safe for the correction of various vision problems. There are, as with any surgical procedure, certain safety issues and risks that anyone contemplating Lasik should be aware of. These are made clear in our consent form  
  How do I know if I’m a good candidate for Laser Eye Surgery?  
  As qualified eye care practitioners we can confirm, based on a comprehensive pre-operative examination, if you are a candidate for Lasik.  
  Will Lasik hurt?  
  Most people feel nothing more than mild discomfort during or after the procedure and no real pain. Because your eyes are “numbed” with eye drops before the procedure, you should not feel pain throughout the Lasik procedure. The application of the actual laser beam should also be painless. There are no stitches involved. The corneal flap is simply laid back into place and it adheres to the rest of the cornea.  
  Could I get an infection from Laser Eye Surgery?
  This is extremely rare, but as with any surgical procedure, there can always be a risk of infection. However, the laser beam itself cannot transmit infectious matter. After the procedure, the surgeon will provide you with prescription eye drops to guard against any post procedure infection. If you use these drops as directed by the surgeon, the risk of infection is extremely rare.
  When can I return to work?    
  Most patients return to work the next day.
  Will I have to limit my activities after Laser Eye Surgery?    
  You will probably need to limit only the most strenuous activities, and probably just for the first few weeks following the procedure. (Remember you should not drive a car immediately after the procedure). Most people return to work the next day, unless your job might cause injury to your eyes before they are completely healed. You should not engage in activities such as contact sports or swimming, where you could injure your eyes as they heal.
  What kind of vision can I expect after laser eye surgery?    
  In the first few days after surgery, your vision may actually be a bit more blurry than usual, and you may be sensitive to bright light. This is because the cornea is still healing after the laser eye surgery. As the cornea heals, the blurring and high sensitivity to light should diminish. Post Lasik eyes are often dry, but drops will help this condition until they return to normal. As for your actual post-Lasik vision, it will be tested, usually the next day or within a few days of the laser eye surgery. Your vision may continue to improve over the first few weeks. On subsequent visits, your eyes will be tested for vision, healing, and to check for any possible side effects.
  Is Laser Eye Surgery permanent?
  Laser eye surgery vision correction is a permanent procedure. Although a small percentage of patients still may require low prescription glasses for certain activities, the vast majority of patients are able to stop wearing contact lenses or glasses altogether.
  If I need reading glasses now, will I still need them after laser eye surgery?
  In all likelihood, yes. Most people who wear reading glasses continue to need them even after laser eye surgery. The reason? They suffer from presbyopia, a visual condition occurring with age, which laser eye surgery does not address.
  If I want to wear contact lenses after laser eye surgery will I be able to?
  Yes, in fact there are certain outcomes that may benefit from a contact lens, e.g. as a temporary bandage lens, or as a temporary visual enhancement, or as a temporary improvement for a particular working distance.
  I’ve heard that some people undergo a second procedure. Why does this happen?
  This is not common, but in no way means your laser eye surgery was unsuccessful. When a surgeon believes that further improvement of a patient’s vision could be achieved, he/she may advise an enhancement.

Every clinic has varying charges. At Kirkwood Fyfe the price we advertise is the price you will pay. We do not charge extra for different prescriptions as the prescription really doesn’t make a lot of difference to the cost during the procedure.

We use a Femtosecond Laser (Z-Lasik) to create the flap and Wavefront Technology during the procedure but these costs are also included in the price we show. In other words we have no hidden charges.

We have been asked often about surgery prices and why there is so much variation. We also learn of those who have been subject to serious upselling and others who have indicated a level of misrepresentation by "Lasik eyes" in advertising laser eye surgery prices that are not correct.........

Why is there so much variation in terms of laser eye surgery pricing?
In a nutshell "You get what you pay for… " Yes there is a level of variation out there but beware , and here are some issues to consider…

"Up selling" or "Bait and Switch". Prices for Laser vision correction are often advertised to look really affordable - almost for the same price as contact lenses e.g. from £395.00 per eye. The reality is that very few if any receive laser vision correction for that price. To be eligible you probably have to have a prescription that is not worth treating! Once you have made the call and have been enticed, in no time you will be informed that the low offer is not available for your prescription and that a higher priced laser eye surgery procedure is necessary. Also to get the best laser eye treatment such as a customised treatment, you will need to pay an even higher price and furthermore with Intralasik laser eye surgery- a far safer procedure you will have to pay even more…. Before you know it you could be spending up to £1800 to £2000 per eye! You might have a few questions such as how do I know Customised treatments are better for me? Or if Intralasik laser eye surgery is safer then why is a blade being used at all? These are questions that you need to research and our advice is to check the market out and see what different organisations have to say.

In other words get "second opinions". Several organizations are upfront and do not discriminate in terms of level of correction and do not "up sell" or "Bait and Switch". They are typically smaller and less commercial. Take your choice…

Clear vision is the result of light entering the cornea (clear window of the eye), passing through the lens inside the eye and coming to a single point of focus on the retina. The most common types of visual distortions are nearsightedness, farsightedness and astigmatism.

Nearsightedness occurs when the cornea is too steep or the eye is too long. This causes light rays entering the eye to bend too much, coming to a point of Focus before they reach the retina. People who are nearsighted have difficulty seeing distant objects.

Farsightedness occurs when the cornea is too flat or the eye is too short This prevents light rays entering the eye from bending enough to come to a point of focus on the retina. People who are young and farsighted can often see at a distance but have difficulty seeing close objects.

Astigmatism occurs when the cornea is shaped like a Rugby Ball, steep in one direction and flat in the other direction. This causes light rays entering the eye to focus in multiple points within the eye, causing double and/or blurred vision. People who are nearsighted or farsighted also commonly have astigmatism.

Am I Suitable?

Because of advances in technology and surgical skills, the vast majority of applicants nowadays, are suitable for treatment in our clinic. We therefore recommend that you book in for a preoperative assessment. In the unlikely event that you are not suitable, you will not be charged for this appointment.

Pre-Operative Assessment

At the pre-operative assessment which takes approximately 1 hour, we use special instruments to take various measurements of the eye to investigate the exact nature of your vision problem, revealing which type of procedure would be best. These include corneal mapping, corneal thickness calculations, IOL Master measurements, and anterior and posterior laser eye scanning. Thus, when the actual procedure is undertaken, we know exactly how to plan your procedure. You will be informed at this stage if you are suitable for surgery

At this visit you will also be shown an Informed Consent Agreement. As with all legal documents, it is important to read and fully understand this document before signing it. You will be able to drive immediately after your pre-operative assessment as we use laser scanners which do not require pupil dilation.

Contact Lens Wearers

If you are a hard contact lens wearer, surgery will normally take place one month after discontinuation, however we will be pleased to fit you with soft lenses, if necessary, for the first three weeks of this period.
